You are trying to insert duplicates into table for column PK_Policy.. Check your data by putting data viewer.., Remove the duplicates before inserting the data.

Hi, Message clearly suggests that, the rows you are trying to insert into POLICY table contains the value for primary key column which already available in table or may the new rows are having same ID more then one time.
