Custom Form with custom Display

Hi All,

My requirement is as below. please provide a best solution with little coding

I have 20 fields
Title
Name
Number
City
State
Profile
......
.....

.....
20th filed

1. I need to create a custom form and when user enters all the fields 

should go to one list.

2. I should provide a 21st filed which should be a link to open the particular item in a new page or a dialog box with all 20 fields displayed

3. Can we have a Library view as shown in attached image which shows only main fields to end user. Below is sample view which shows data for 6 items.

Hi Navy007,

If you have a SharePoint Enterprise license then you can do all that you asked for using InfoPath Designer.

There are several InfoPath designer blogs out there.

I myself  have published several TechNet articles where I have built InfoPath forms with similar setup that you described.

you need to have your prototype as HTML first then you can either modify the XSLT of the list viewer webpart 

below is an example to customize listview webpart

Hi Amr Fouad,

That was really Nice. 

But I need the view in 3 columns ( for example if I have 15 items then i need 5 rows with 3 columns each) and also need to retain the header so that i can filter the items.

Let me know if you have any suggestions. Again thanks for your solution.
