Hello,
Why is there free space in a mailbox database, is the free space created when items are deleted ?

Its the logical space within a store that is available for Exchange to reuse when messages are deleted and purged from the database. Before 2010, Event 1221 in the app log would tell you how much free space was available.

Because not all the space in the database is occupied by data. To be
so efficient with data storage that there isn't any free space in a
database would require extraordinary effort. The machine would spend
more time uselessly shuffling data than it would in accomplishing the
job it was supposed to be doing!

That's one way. Another is when you add data and the file grows to
accomdate that data. The file grows in increments larger than what's
needed by a single message and the unused space is, well, free space.
