I am trying to create an outlook form that will do the following:

1) Allow the sender to attach multiple files and comments.

2) The reader will see each attachment in a column with two radio buttons next to it (approve, reject) and a comment box.

3) Button to send results back to original sender.

I am having trouble finding help on how to get the attachments to work the way I intend. 

If this is possible, any help/guidance would be appreciated. Thanks.
