Increase the Diagonostic Logging on Default SMTP VS and check for the logs. and see where it is going wrong. also make sure the recipient is always same everytime? whenever this issue happens?

Check info:
1. Whether the issue occurs intermittently or permanently for problematic senders?
2. Does the issue happen to new composed messages or replied messages?
3. Quote: Its happening only for internal emails
Does it mean that the mails from problematic wont be duplicated for external recipients?
4. Can the issue be reproduced via OWA?
5. Which protocol do users use, exchange account (MAPI), POP3 or IMAP4?
6. Is there any similarity among the problematic senders, like same mailbox store, or same exchange server?
7. Are the problematic recipients also the smartphone (BlackBerry) users?
8. Please disable all the 3rd party software temporarily and test the issue, including the Anti-Virus software
