We received the error on Exchange 2003 very frequently. (Source: COM+ and event ID: 4786 "The system has called a custom component and that component has failed and generated an exception.") I find the same on Microsoft but MS recommend me the install the HOTFIX http://support.microsoft.com/kb/885422. I got the message when I install the hotfix.
