InfoPath form in SharePoint 365 with workflow for approval and to create tasks and calendar items

Hi,

 

I would like to have a form within SharePoint that fits the following description:

 

Content in the form:
a. Their Name (User)
b. Their Position.
c. Dates for beginning and end of the planning period.
d. Their supervisor (Another User)
e. User-generated performance targets with dates assigned (labels and numbers)
f. Categories of the targets (some categories come from their job description, others are universal)

g. User-generated Descriptions associated with the targets (text)

h. user-generated dates for four future review meetings

Workflow
o Form initiated by individual, routed to Supervisor.

o Both can edit it.
o Supervisor can approve plan, at which point it cannot be edited.
o Approval of form creates tasks for each performance target and date for the individual; creates meeting for supervisor and individual.
o Data is stored in a way that can be pulled into "Quarterly Report" forms that would show what performance targets should have been achieved and what actually has been achieved.

 

Permissions:
o The form can only be viewed by the individual, their supervisors, the supervisors of the supervisors, etc.

o The form can only be edited by the individual and the supervisor.
