I have a problem as below:
When it working fine, whenever i deleted the file Msforms.exd in C:\users\username\Appdata\Microsoft\Forms then run an Excel macro file, this file will be automatically re-created.
Since i used command regsvr32.exe /u c:\Windows\sysWOW64\FM20.dll and regsvr32.exe without /u to register again. Afterward, issue came up, i found out the file MSForms.exd no more automatically re-created in that folder, so i suggested this missing something and caused the issue.

Do you mind tell us the detailed info about Excel version? I also tested with my local environment (Office 2013 15.0.4693.1000 & Windows 8.1), but I can't reproduce it.
Please let me know the result.
On the other hand, the Form Controls (FM20.dll) are no longer working as expected after installing MS14-082 Microsoft Office Security Updates for December 2014. We may also remove the updates and check if it works well.

Hotfixes for this issue have been released in the March 2015 Updates for Office 2007, 2010 & 2013
