Ms Outlook 2013 was offline, calendar items entered offline, when Oulook went online, all off-line calendar items are now gone.

When you are using Outlook in Online mode, the mailbox data would be directly synced with server side data. The Calendar items you entered offline have not updated in mailbox on Exchange server immediately. Therefore, there is no offline caledar items in Online mode.

I suggest we can use Outlook in Exchange Cached mode. Then working offline and entern the calendar items. Close Offline, the offline entered items would be saved in the cached profile and updated to server mailbox for some minutes.
