1. What are the important things to be considered for SMTP flow inbound/outbound

Ensure that port 25 inbound/outbound is open in your firewall 

Create necessary policy elements to filter incoming\outgoing emails in your Barracuda to tighten the SMTP security which is very imp 

2. If i dont want CAS url to be published on ISA/TMG or any other Hardware appliance. Can i directly publish CAS url using public IP and register the A record on my public DNS and Private DNS.

I will not recommend this config since its not secure and its not the best practice. Always publish them through a reverse proxy

And what changes i have to do on firewall to accept port 443 traffic and route to the CAS NLB

Open port 443 in your firewall. Create a rule in the firewall to route all the webmail  traffic to your CAS VIP(if its CAS array) or CAS server then its CAS IP

Can you please describe the difference between the reverse proxy and the simple configurations i have mentioned. (Public IP NATing to CAS NLB VIP), what are the demerits of this setup.

Since i do not have budget for reverse proxy what other substitute i can adopt for publishing CAS URL.

Also let me know if i have to request for A records config at public DNS, resolving CAS URL to Public IP.

I have a Exchange 2010 setup which is not published public. 

i.e. users can't connect to the CAS from outside network and can't send/receive emails externally.

Now, I want to configure the setup in such a way that users should be able to send/receive emails externally.

For that i have purchased Baracuda ANti-spam appliance.

Kindly provide me inputs for the points mentioned below.

1. What are the important things to be considered for SMTP flow inbound/outbound
