A CAL isn't tied to any e-mail address. A mailbox can have as many
e-mail address as you want (within reason), and you can have as many
mailboxes as you like. A CAL is associated with a person and/or a
device.

Hello,
A CAL is not a software product; rather, it is a license that gives a user the right to access the services of the server.
So, CAL is not required to add email addresses on Exchange 2003.
You can know CAL more detail from this document:
Client Access Licenses and Management Licenses
https://www.microsoft.com/licensing/about-licensing/client-access-license.aspx
