Hi,
You have specifically added Send-As and Full mailbox Access, but you have to add user yy to GrantSendOnBehalfTo property of user xx on Exchange Server 2003 if that property exists .
In Exchange 2007 I can do it using EMS:
get-mailbox xx | set-mailbox -GrantSendOnBehalfTo yy

Hi Richard,
Would you please let me know whether you have restarted the Information Store service to clear cache after removing Send As permission and set delegate permissions?
If the issue persists after restart IS service, I would like to explain the issue may occur:
1. The user yyy belongs to some groups which still have Send As permission to user zzz
2. Some AD replication issue persists.
To troubleshoot the issue, I suggest you create a new mailbox user and grant the user delegate permissions to the user zzz. After that, please check whether the issue can be reproducted. If the issue cannot be reproduced, please compare the new created user and the user yyy AD attribute to check whether user yyy belongs to some specific groups.
Thanks,
Mike
