After upgrading SCSM 2012 to R2, DW Server stuck on Implementing MP "Microsoft.EnterpriseManagement.ServiceManager.UI.Administration" in running state and Receive "MonitoringHost.exe process crashes" events

any one faced such problem, any help is thanksfull

Did you ever get in resolution on this?  I'm seeing a very similar issue.

Did you follow the upgrade documents?

There is a particular issue that "The data warehouse stops working after upgrading Service Manager in System Center 2012 SP1 to System Center 2012 R2 due to a Data Warehouse fact entity upgrade."

If you follow the exact steps and run the SQL query to prevent this issue, you should be right.
