Windows 8.1 showing wrong taskbar icons

I am getting the wrong icons on my taskbar, it's always different apps that are effected both with wrong icon and whose icon is used.

Right now you can see powershell has the one note icon.

Have you tried to clear the IconCache?

yes i know its a issue on my system. But it's an issue on my HTPC\Laptop\PC\Virtual machine so its a common problem even if you have not had it.

I'm having the same issue on my desktop computer (windows 8.1), it has 2 screens, the icons are wrong in different ways on both screens, see screenshots below.
I also own a laptop that does not have this weird problem.

Spotify clearly should not have the Opera icon (wrong on both screens) and the Snipping Tool icon is applied to the Remote Desktop Connection on the left monitor, on the right monitor the icon is correct though.

I've never had this issue before (on windows 7 & 8)

This solution did not fix the problem. I did not have this problem until I upgraded to Windows 8.1 a couple of days ago. Now when I open Adobe Reader, the Photoshop icon displays in the task bar. Really annoying. Purging the icon cache did nothing.

I have exactly the same issue. PS icon showing for Acrobat. Annoying.

I have had this on my 8.1 system for about 6 weeks now, it gets really quite tied up in a knot sometimes, more than just the icons changing, the links seem to change as well. CTRL-ALT-DEL and log off clears it all back to normal again.

I've been annoyed by this constantly happening for a long long time.  Checking up on it again this last time, I found an article relating the issue to small icon mode, which I use.  I discovered that at least the problem can be fixed temporarily WITHOUT REBOOTING/killing explorer.exe by unchecking "Use small taskbar buttons" and then checking it back again.

Tried all the solutions in this thread and none worked for me.  Acrobat and SQL Data Compare are both showing a completely different app icon that isn't even open!  Clicking the "use small icons" appeared to clear out the icons as everything went blank but as they filled in, they still had the wrong one and unclicking it produced the same effect.

I've had this on multiple windows 8.1 machines and even one a freshly rebuilt one.  Nothing appears to be fixing it and I have no idea what program i'm clicking on without waiting for the desktop preview window!

The daft thing is, the preview window gets it right!!

I have this problem too. For example, Acrobat Reader shows the Serif PhotoPlus Icon in the taskbar. Like all above, this has only happened since upgrading to Windows 8.1.

I even went onto the Microsoft Chat. They had the cheek to tell me it was a fault with my computer, and I would have to PAY to get an answer!

This is clearly a problem with this version of Windows. Any help resolving it would be appreciated. Thanks.
